Part 4， 25 ～28 Information about beneficiary family, provide information about the beneficiary’s spouse and children.

   父親已去世，還需要把父親的信息填上去嗎？

-Not need. Just choose "Widowed" in Item 18 of Part 4.

或填上父親的信息並注明去世？

-
