My dad passed way, my mom is here with me now with B2 visa; I have a brother in China now

the notary I have is worded like this:  

xxx/name, born in xxx/year, month, date; 身份證號碼 xxxxx; is my mom;

xxx/name, born in xxx/year,month, date  身份證號碼 xxxxx; is my dad;

xxx, born in xxx /year,month,date , 身份證號碼 xxxxx; is my brother;

Is this good enough?

And if my mom marriage status is windowed , I don't need to fill out any information for my dad in the part of her marriage section, right?
