尊敬的８老師， 如果在美有過被捕經曆，除了Part C, 1, b,  選 “yes” 外，是否還應該附上犯罪記錄， 並且寫一份 statement?

-Yes, explain the case and provide all of the related documents (photocopy) from the Police Department, Court,, etc.
