請教父母移民130 RFE中，Booklet issued by the Province's Health Departme

父母移民，收到要求提供幾個額外證件：
1.Birth Certificate
2.Hospital Certification
3. Family House hold registry
4. Family House hold register form
5. Booklet issued by  the Province's Health Department regarding the birth.

1和2肯定沒有，因為我沒在醫院出生。戶口本的話，我的和父母不在一個戶頭上了。關於4，公安局那邊說原始的戶口登記表都查不到了，因為時隔太久了。那個5是什麽意思？國內衛生部有每個人的出生記錄嗎？
