請問8老師，父母境內申請綠卡中，現在要求再次出示父親和我本人的出生證明

信裏主要內容是要這個：
Chinese Notarial Certificates are considered to be secondary evidence. Therefore, at least one of the following official documents must be submitted in support of the notarial certificate of birth:
- Birth Certificate
- Hospital Certificate
- Family Household Registry
- Family Household Register Form
- Booklet issued by the Province's Health Department regarding the birth
我父親的證明應該就是戶口本吧？
但是我本人的戶口本那一頁還是出國前的紀錄（也就是說是錯誤的信息），這樣也能公正戶口本嗎？
另外我本人的醫院出生證明沒有我的名字，隻有媽媽的信息，和記錄何時生產的一名嬰兒。 是否我重新讓醫院開的出生證明裏需要有父親的名字？
現在很著急，因為要87天把證明寄給移民局。多謝幫忙！
