My hu*****and already died.
 

So for Current hu*****and, I should write none.
 

For former hu*****and, I should write his name, I also need to fill the blank for "date and place of termination of marriage". How should I fill the blank?
-Put "Deceased" there.

Since we did not divorce and I have been widowed since his death.
