For G325A for my mom greencard application:

1. "Applicant's residence last five years": for over the past several years, my mom has come to the U.S. many times. Therefore, the space for this question is not enough.

-If your mother' stay in US was > 6 months, then put the US address there. Otherwise, not need to put it there.

Do I need to attach a piece of paper to add more information for this question?

2. "Applicant's last address outside the U.S. of more than one year": after 2007, my mom never continuously stayed in China for more than one year. So I need to list my mom's stay before 2007, right?
-Put the address in China before her last entry to US.
