I am a U.S. Citizen and applying Greencard for my mom. I have the following detailed questions.

1. I130
1.a Question C8 (Date/place of present marriage), C11 (name of prior spouse), and C12: my dad passed away long time ago. My mom is now widowed and never got re-married. So I should leave C8 blank. How about C11 and C12? Also leave it blank?
-Yes, just leave it blank.
 
1.b C17: “List spouse and all children of your relative”: do I need to include my dad information here since my dad passed away? If I include my Dad information, under column “Relationship”, do I need to note “Deceased”?
-Yes, list your father's name and put "hu*****and" in the "Relationship" part, and put "PR China. (Deceased)" in the "Country of Birth" part . Also need to include your name there as your mother's child.

2. G325A for my mom
2.b For question “Current hu*****and or Wife”,  and “Former hu*****and or wives” , do I need to leave this question blank for my mom?
-Do it as C17 of I-130 above.

3. I485 for my mom
Part 2, Application Type:
I should check “h”. because I plan to file I130 and I485 concurrently, I will put “FORM I-130 BEING FILED CONCURRENTLY “ here. Is this ok?
-No. She needs to select "a" even if concurrently filing I-130 and I-485.

 
4. I864

4.a I am applying GreenCard only for my mom. My income alone is well above the poverty line for my household. So I do not need to list my wife income, right?
-Correct.

4.b If yes to question 4.a, for question Part 6 “Sponsor’s income and employment” , 13.a.1, is the total income “my own income” or “my income + my wife’s income”?
-Your personal income.
4.c    If yes to question 4.a, I can leave Part 7 (use of assets to supplement income)) blank, right?
-If your salary/cash income is enough, just leave it blank.

4.d My wife and I filed joint tax return every year. I just need to attach my own W2 and our IRS transcript for the past three years. (The income listed in IRS transcript is larger than my W2 income because of joint return, do I need to include my wife’s W2)?
-Just provide the copies of your personal W2 and the joint tax return.
