Please: which address to use

1. The address (Address A)on the employment verification letter head is different from the original recuit branch address, also different from the address (Address B) for PERM and I-140 filing.

 Which address  is the best for my I-485 filing? Address A or Address B?

2. Both of the above addresses are far away from where I work (different states). Is it OK?
