What happens when 485 medical exam fails?

Our pediatrician recommends that my wife not take MMR vaccination due to breastfeeding our baby girl. The civil surgeon has tentatively marked "not pass" on the medical exam but put pediatrician's comments there.

We argued for a potential waiver of MMR but it was denied as CDC states that MMR is not a contradition for nursing mom although it says cautions need to be taken.

My question is: when USCIS sees "not pass", will they simply deny the I-485 or will RFE?
