多謝8老師，我就是從http://www.flcdatacenter.com/  找到的prevailing wage, 這個說是OES, 2011.

所以我填表就寫了  

1)Agency which issued prevailing wage: N/A
2)Prevailing wage tracking number (if applicable): N/A
3)Prevailing wage source: OES  (這個OES是四個選項之一）
4)Year source published : 2011
