more information requested by USCIS for i485

I recieved the letter regarding the information below:

1) Birth certificate: As we all know, we don't have birther certificate in China, I might still have the notary that including my name, my parent's name, but it was done in 1999, can I use it? I tried to find more info on state.gov, but no luck so far.

2) Med exam and Vaccination Record: Same here, we don't keep vaccination record when I was in China, how do I deal with this?
