question about DS-3032

I am applying GC for my dad who is living in Beijing right now. The I-130 got approved. I got DS-3032 today, but I am not sure how to fill it out. We don't have a lawyer or agent. Should I put my US address and my dad's name? The "Signature of Applicant" should be my dad's signature, right?
