I am on H1B. I applied for EB2 in 2009, got labor certificate and I-140 approval, but have not submitted I-485 because the priority date is not current.

I just married to a US citizen and want to apply for family-based green card. I have following questions:

(1) I have an A# on my current h1b I797A approval notice and a different A# on my I-140 approval notice. Which A# should I put in when I fill out I-130, 485 forms, etc. for family-based greencard? Or should I leave A# blank and let USCIS decide?

(2) When I submit I-130, 485 forms, etc. concurrently for family-based gc, do I need to include the following materials: h1b I-797A approval notice petitioned by my previous employer, h1b I-797A approval petitioned by my current employer,and I-140 approval notice petitioned by current employer?
