how to fill out the ds-3032 form to change address and agent on

Hi,

Could you kindly share how to fill out the DS-3032 form:

Shoudl I put the petitioner's name or beneficiary's name?

It also require to put case barcode strip on the form. Which one should I use? The one form the receipt cover page or the one from the fee notice letter from the NVC?
