1.PART1, Individual named in the related case, 是填我的名字還是公司人事部頭頭的名字？
-Leave first line blank, and fill the company name in second line "If filed on behalf of a company"

因為I-140是以公司人事部頭頭的名義填的。
-Fill 公司人事部頭頭的名字 in the line "Name of Company Contact"

2.PART2, BLOCK 4,Petitioner/Applicant 應該填誰的的名字？
-Your company's name.

3.PART2, BLOCK 5, Benificiary 是不是填我的名字？
-Correct. Fill your name there.
