-Why do you say the I-94# is wrong? The I-94# can be the same or different after you extended your status within U.S.

and my wife's birth day (one number of year) is not correct after several times of H4 extension. Do those get any trouble during I-485 approval?
-You just need tpo provide the correct birthdate in the I-485 application. If you get RFE for this issue, just explain it.

Can I only the original one when they enter USA?
-No. you need to provide all the copies of I-94 cards and H1/H4 approval notices since your latest entry into U.S.
