Help pn Procedure of applying GC for parents

Following Pingjiang's suggestion, I am going to apply GC for my mother. Her I94 will expire in one and half month.

I am not clear the document lists and procedure to do this. Is anyone can pointing me the link of USCIS to give me the whole lists of documents?

Where can I find the qualified doctors to do physical exam?
