you have to write a request letter to USCIS, indicating you are going to link 485 to newly approved 140, and attach all the documents: old 140 receipt/approval letter, new 140 receipt/approval letter, 485 receipt and your ID copies et al.
Mark your envelope with bold letter "485 relink" and 485 receipt number. Good luck
