sorry for so many questions. Do i need to enter my spouse's name and address and check petitioner, applicant,and beneficiary? I am looking at g-28 form.

also, do i need to enter law firm's name when I terminate the services (panel 4 under 'Check Applicable Item(s) below:')? if I enter only lawyer's name, are other lawyers in the law firm continue to have access to my case?
