Question on Individual Taxpayer Identification Application

I am filling the form to apply the individual taxpayer identification for my mother and having two questions.

In the reason section, which one should I check? b: Nonresident alien filling a US tax return OR d: Dependent of US citizen.

My mom has B2 visa so her only document is her passport. Do we apply in person in the tax assist center?
