It needs to be properly documented on the form.

starting late 2006, NSC asks for TB skin test regularly, and after Aug 1, 2008, TB skin test is a must, unless you don't need one according to civil surgoen's manual. In that case, doctor needs to write down why you don't need one citing the regulation.
