EAD renewal Immigration Status

I am renewing my EAD. Currently using EAD working, company is applying for H1-B for me. I-485 Pending.I filed I-485 on July 9th 2007 and the receipt date is Sep.19th 2007.

What should I put for "15. Currently Immigration Status" on I-765 form?

Do I need to pay $340 filing fee?

Do I need to submit copy of my old I-94?
