How to choose

Dear Friends,

I am filing I-485 for my mother who is currently visiting us. I do not know how to choose "Application Type" in I-485 Form. Your advice would be deeply appreciated.

I am also filing I-130 for my mother but don't know how to arrange physical examination for my mother.
