Help!! waiting online....About TB test on RFE

Today, we got the long-awaited (15 days since I got email of the notice of it)RFE letter finally requesting for TB test.
I called the civil surgeon and made appt on next Tuesday. They asked me to take all my paperwork of examination taken previously (around one and half years ago) but I don't think I have any copy of that. I remember last time they just gave me a sealed envelop which I handed over to my attorney straight.
1)Can someone who has been requested for TB test tell me if the paperwork of your medical examnation taken before is required in this TB test. I am worried they are gonna ask me to do the whole examnation again without it.
2)how long does it take to get the result from the doctor?
3)when should the form I-693 be filled by the doctor? Should I bring it with me to the TB test appt?

I have to give the test result back to attorney asap before the dealine.
