I-485 refile

我剛收到移民局要我重新遞交個別文件的信。要我遞交：

Submit the following evidence to verify the birth and parentage:
1。 A photocopy of the Household or Village Registry reflecting the entry of me.
2. A photocopy of the booklet issued by the province's health department reflecting my name, gender, date of birth, and both parents' name.

我想請教：第一個問題是不是要遞交一張類似全家福的照片？關於第二個問題，本人不記得有過出生證 (生於1970)。是不是要戶口本？可是我在國內時是集體戶口(畢業後在北京工作)，戶口本不在我手裏。我也不清楚自己的戶口本在哪裏。
