H1B 第一個3年12月到期, 140已批, 485 pending, HR is preparing the H1B renewal for me

1. 表格part3(Information about the person(s) you are filing for.)的第1項中 A#

Should I input the A# from I-485 receipt?

2. 表格part4(Processing Information.)的第7項 Have you ever filed an immigrant petition for any person in this petition? If Yes - explain on separate paper

Should I answer YES for this question? If so, how can I explain on separate page? Just list when I-140 is ted and approved and attached the copies of the receipt and approval notices?
