pjiang: please help with remark in medical exam result.

I took the medical exam and in the copy of result, the doctor did not check for "current for all the immunization". The doctor explained that I had no vericella and she just gave me one shot and I need to have 2nd shot in one month. In the remark of the result, she wrote "need #2 vericella shot".

My question is if the examination result is OK?

Will I get a REF for have take the 2nd shot?
