help: Regarding 485 Form

I just got my EB1A approved. Now I am planning to file 485. Questions:

1. in Part 1. What does Address-C/O mean?

2. in Part 2. Application type: Should I choose "a" and my wife choose "b"?

3. Should we leave Middle name blank or N/A?
