I485 RFE TB Skin Test (urgent)

Hi, everyone. I receive the RFE, saying that "The medical examination (Form I639) contained in your file does not indicated that the required TB skin test was conducted. You should return to the civil surgeon who first examined you (another civil surgeon authorized by this Service) to properly complete the Form I639 medical examination to include the required TB skin test.

As read a similar notice in the forum before, but can not find it now. So I just need go to a surgeon, and ask for TB skin test. I believe it is positive, that is why I took the X-ray examine when I ted the appllication years before. So I ask the surgeon to write something to explain the "positive" is due to childhood's vaccine? Best regard, I am waiting for some advice...
