questions about form I-539 F1 -> H4

1. part 4 additional information-- 3g-- I should choose yes or no?

I never change of my status before, always F-1, but transfered once from one
universtiy to another. I did TA most of the time. so how do I explain?

2. it requires the marriage certificate, is the copy of marriage certificate
enough (we have US marriage certificate)?
