xiaobaitu and other experts, can you come in please? EAD filing

I am EB2(LC based) 140 approved and 485 pending in NSC. I will need to apply for EAD now. Is the following all the necessary materials?
1.Form I-765
2.I-485 receipt notice
3.Copy of previous EAD card
4.2 photographs

Is that all? Did I miss anything?

Also, another question: if I don't use my EAD after I apply for it and still be able to keep my H1-B, then I don't need to renew it before it expires, right?
