Q1: Probably 學校的地址 is better. Your parents' address would be your permanant address in China. 具體到哪個building, room is not necessary, but if you still remember them, it won't hurt to write them down.

Q2: NO problem with "OPT卡上的start date就是我的job start date" as long as you dont' work before that. This way you actually have longer time to work.
