Xiaobaitian, please help on how to get vaccination report

Xiaobaitu, Would you please help me for the following two questions:
1. How to get vaccination report since I do not have any vaccination record?
2. For medical exaination Form i693, Should i take medical examniation before applying 485 or after ?
