About H1B visa stamp 請教小白兔及熱心人

My company will help me get H1B in premium process. Since I am in Canada now and this is my first time to get H1B visa (I have B1/B2 now), I would like to get the help from you.

1. Once my H1B is approved, how could I get my H1B visa stamp? From the boarder? Do I need to book an appointment in a Consulant to get it?

2. How long is it to get a H1B visa in premium service? Longer than 15 days?

3. In the application, I am asked to give information regarding to the immigration history (every type of US immigration visa is held and dates). I got confused here. I have B visa and a volid I-94 card (till 04/2007), I went to interviews with B visa and stayed in States for a few days. So could you please tell me which dates I should fill here, the length of my B visa, all days I visited in USA with B visa or the dates on my I-94 card?
