Xiaobaitu: change of job title for H1b?

My questions regarding job title change with the same employer:

1. Can my employer file the extension of H-1b and change job title together or has to be done separately?
2. Does the change of job title need to go through Dep. Of Labor first before filed to USCIS?
3. Does it(change of job title) require approval from USCIS once filed or is it a just a notice of job title change?
4. How long it takes to process?
