Questions on Eb1 forms

I am filling 140 for my Eb1.

As I had filed another 140 as NIW previously, for part4 Question 6, should I check yes and print case number etc. and attach to this 140?

For 485:

Part 2, which one should I choose ? I am on my H1 and use concurrent filing.
Part 3, should I write down the service center (NSC) or the place where I live and file my 140 petition?
