I-140 just approved at TSC, question about I-485

RD: Aug 23,
LUD: Aug 25, Oct 17.
Aproved: Oct 18.

I got the approved notice from my e-mail today. Can I begin to make an appointment for the medical check right now and how to choose the clinic? As my wife and I work in the different cities,how can we deal with the situation?
