We just included following items and got approved yesterday.

1. Completed I-539 form
2. A check of $200 for the I-539 form filing fee
3. Copy of her passport, I-94 form (front and back sides).
4. A letter from the employer of XXXXXXX
(to approve you have financial support for him/her)
