I went to somewhere for a conference or a vacation (don't remember, but last year 2003). I flew out of Detroit, and flew back in Detroit (I think). The airline people forgot to take it when I went out, and I realized this when I boarded the plane. I didn't want to give it to the airline people on the plane, because I didn't want them to lose it or something. When I came back in, I didn't want to hide it, because I don't know if the immigration has it in their file that I haven't left (because I didn't surrender the I-94). But I think they don't have this information on file--if this immigration guy didn't see mine, he would not have known that I didn't submit it on my way out.
Hope I'm making myself clear... What's your situation?
Also, I don't think I-94 you have to apply each time? At least I never need to apply. I just take the form the airline gives me and fill it up before I enter U.S.
