J1 waiver materials to submit

Hi, I have a question about J1 waiver, please help, thanks.

Afer I get the case number from DOS, what kind of the documents they will demand me to show, do I need any document from chinese embassy at that time? I want to apply for the number next week, before I do it, I want to know this first, pioneers please help with.
