I am a little confused. How does one know if he needs to go out of the US to get an H1-B visa or not?

If one needs to go out of the US to get an H-1B, how about the extension of H-1B, when 485 is pending?

Under what circumstances, can one switch to H-1B e.g. from F-1 without leaving the US to geta visa, and in what cases should one do that?
