Date Visa was issued: the visa that you used to enter US last time. Not the date that H1_B was valid (it seems you don't have a H1B visa, what you have is H1B status)
Also, on the line above it there is "Nonimmigrant Visa Number" - NOT I-94#. Check you US visa you used to enter US last time, there is a visa number.
