Xiaobaitu, are you there? urgent question on I-485 form

I have to finish the I-485 form and mail it out tomorrow, so please help if you know the answers.

In the form I-485,page 2, Part 3A, there is a line asking to put in "Date Visa was issued". Is that the visa that I used to enter US last time? Or is it the date that H1_B was valid? My attorney put in two different dates in different copies, I don't know is that a mistake or is that the way it should be.
Also, on the line above it there is "Nonimmigrant Visa Number", is this the I-94#?
