On my application I attached a separate sheet of paper as following:

Form I-131 Attachment

A#: Axx xxx xxx
Part 7: Qualification and circumstances warranting issuance of Advanced Parole document…
I have an adjustment of status application pending and seek to travel abroad (my homecountry) for personal reason.

AP was approved within one month.
attach a copy of I-485 receipt is important.
