1. As long as CIS receives the filing before your current H-1B expires, you are OK. You usually can do it 3-4 months in advance for safety in case something goes wrong with the application.
2. Usually No, your employer owns the application. And they need to file the extension.
3. from expire date.that is expire date+3 years
