If I notify INS to send the notice to me, which form should I fi

My lawyer filled I140 and I485 concurrently in the beginning and both of them were denied. I appealed my I140 by myself and it was approved.I am afraid the formal notice would be sent to my previous lawyer and want to send a letter to the INS. I do not know if a letter is enough or I should fill a specific form.

By the way, I also want to restore my I485. I think you are very experienced. If you happen to know which form I should fill, would you please let me know? Thank you very much for your information.
