45 days letter from Phila NSC received

My LC app was submitted in Jan. 2005. I just received the 45 days letter from Phila NSC this week. The letter asks for continuation option and missing form ETA part A&B. Why missing form? does anyone know what's going on here. What does this letter mean? Is it good news? Thanks.
