What's your visa status? If you are J1 in clinical pratice, yes, you have to be employed by certain designed employer in underserved areas for 3-5 years. If you changed from medical research to medical practice after filling I-140, you are lucky to get I-140 approval, but you still have to fulfill that requirement to get GC.
