Apparently you are a physician, applying for green card on the term that you will serve in the underserved area in US for 5 yrs. Congratulations that your 140 is approved, but until you have fulfilled the requirement to work in the underserved area for 3-5 yrs your 485 can not be processed. This is not a typical approval, only specific to your case.
